I've helped finance many deals in this space, and I believe these are two factors to consider: This is a strong industry, but growth is limited by caregiver supply rather than demand. Home health and personal care aides are among the fastest-growing U.S. occupations, yet annual turnover often exceeds 60%. That makes staffing a key diligence item. Agencies with clear retention systems such as competitive wages, guaranteed hours, and consistent scheduling produce more stable cash flow and underwrite better with lenders. Home care agencies can be private-pay, Medicaid, Medicare, or a mix, and payer mix materially affects risk. Medicaid-heavy agencies face state rate changes, payment delays, and audits, making cash flow less predictable. Medicare is generally more stable than Medicaid but still compliance-driven. Private-pay offers the most predictable cash flow and is preferred by lenders.. This is not theoretical. We have seen one SBA lender decline an otherwise solid deal solely due to heavy Medicaid reliance, even with strong revenue and SDE, because too much of the cash flow was outside the operator’s control.
