Getting started with lawyers/accountants

Anyone have good suggestions for first steps to finding a good lawyer and accountant to work with? Just starting to set up my LLC and could use legal advice.

1) How much does location matter? Should it be state specific or is it alright if they're anywhere in the US?
2) Do most small-scale legal firms charge similar rates or does price vary greatly?
3) Do people generally establish 1 relationship with a lawyer and then use them continually or is it more fluid/as needed?

Lawyer location doesn't matter for the most part unless you're in a regulated industry, have real estate, licensing, or labor law issues. Legal pricing does vary but generally falls in the $300 to $500/hr range unless you go to Big Law.
