Medical/Wellness Looking to Sell - and remain involved or not

I'm looking for doctors, dentists, behavioral specialists and other wellness providers with revenue $1 million+ interested in joining a national network. It is a roll-up with the ability to significantly improve profitability. They are targeting a sale to a major Fortune 500 buyer on the backend, with 6-10X multiple on revenue. The typical private sale for these types of practices is 1.5-4x EBITDA. So, there is significant upside, and the existing owner can choose to remain involved or not after the sale.
